  • Assist in the coordination of care by working as a liaison between you and your medical team.
    • This coordination can be in the emergency department, in the hospital or for an upcoming procedure. If you have multiple medical providers, require multiple services, schedule coordination or have complex healthcare needs, we are your solution
  • Health care plan and medication adherence management and compliance
  • Prepare you for upcoming doctors’ appointments
    • Review of your medical problem list
    • Discuss your current status and progress
    • Prepare a list of questions, concerns, documents and updates for your provider
    • Communicate information to your provider prior to appointment
  • Accompany you to appointments with healthcare providers, procedures or services
  • Making bedside rounds if you become hospitalized
    • Review hospital plan of care and treatment
    • Assess, identify and address any safety concerns or issues
    • Answer and address your questions/concerns
    • Coordinate your discharge plan and needs
    • Coordinate transition from hospital providers and services to new and/or existing providers and outpatient services.
    • Ensure post discharge continuity of care, referrals, medications, follow ups and scheduling.
